You know, I had an awesome origin for these three mapped out -- only to rediscover that, once upon a time, they were PCs I was playing in a D & D game! Ladies and gentlemen, meet one of my oldest D & D Characters.  I started her in 1st Edition, translated her into 2nd, and only got to play her for three sessions of 3rd edition. Normally I wouldn't have had any information about her beyond a picture, but I was cleaning a closet at home yesterday and FOUND her and almost all her character notes!

Meet Mystique.

Mystique started her career as a first level Wizard in First Edition and when she started out, she had only one head.  Her Strength and Intelligence scores matched -- 17 in each, I remember that -- and the rest of
her stats were more-or-less average.  When she ran out of spells, she'd run to the front lines and smack enemies over the head with her staff.

Anyway, the DM at the time came up with a mega-dungeon which he called  'The Shadowed Halls of Forgotten Lore'.  Among the things we found was something called an Infusion Circle, which we determined would enhance magical items, if we were willing to spend XP on it.  At the time, Mystique was level 5.  She had acquired a Potion of Hydra's Health which would give her 24 hours of regeneration.

being only 13 at a time, I wondered what would happen if I spent a lot of XP boosting this, so when it came to my turn, I spend FOUR LEVELS worth, figuring I'd get it back quick.  The GM was so impressed that when he rolled on his random dice table, he added a whopping 80% since I spent so much.

Basically, Mystique got the ability to regenerate at *checks notes* 4 hp per round.  Unfortunately, there was a sliiiight side effect; whenever she reached zero, something weird would happen to her and provided the damage wasn't fire she'd regenerate.  And one of those weird things was 'grow an extra head'  Hey, every extra head added one extra spell per level so I was happy.  Another near-death and she grew a second head.  A third death...and each head got its own personality.  Worse, they didn't always get along.

According to my notes, by the time I stopped playing her in 3rd Edition she had seven head, a lewd tail (you heard me; it would poke and prod other characters against her wishes) and a pair of wings.  Then I stopped playing D & D entirely and started playing Champions.

And the rest...is history.

Pathfinder, 2d or 1st Edition?  because tehir different and I'll warn you the 1st Ed and 2nd Ed sheets are REALLY kludgy.  We made up a lot of stuff on the fly.  As for her special abilities, that's easy:

Multi-headed Surceress (1st and 2nd Edition)

-- Regennerates 4 hp/round unless hit with fire or acid, in which case does not regenerate that round.  Character can only be permanent slain if rediced to negative CON by fire or acid.
-- If target is reduced to 0 and can still regennerate,  target suffers a side effect rolled by the DM  (Note: I never had access to that table, you'd have to make it up).

Stats (1st ED): STR 17, DEX 13  CON 12  INT 17  WIS  8  CHA  15

Side Effects  (1st Edition):

-- Grows Second Head: -2 to being surprised and +2 initiative.  +1 Spell known/can can cast per level
-- Grows Third Head:  additional -2 to being surprised and +2 initiative.  +1 spell known/can cast per level
-- Different personalities: If the player acts indecisive or takes longer than 10 out-of-game seconds to decide on an action, the heads conflict with what the next action will be.  Character must save vs paralysis (target number determined by Dm based on level of indecision and potential actions) or spends round arguing.

Side Effects (2nd Edition)

-- Wings: flies at speed equal to movement rate, cannot use in enclosed spaces
-- Gliding Wings:  Reduces distance fallen by flight speed
--  Tail: Gains a 1d4 additional Tail attack
-- Tail:  Tail has a mind of its own.  Playful, misceivous, and amorous
-- Grows Fourth Head: additional -2 to being surprised and +2 initiative.  +1 spell known/can cast per level
-- Grows Fifth Head: additional -2 to being surprised and +2 initiative.  +1 spell known/can cast per level
-- Indecisive: -2 to  save vs Paralysis
-- Grows Sixth Head: additional -2 to being surprised and +2 initiative.  +1 spell known/can cast per level
-- Long Tail: Tail can extend an addition 5 feet.

Side Effects (3rd Edition)
-- Grows seventh head: additional -2 to being surprised and +2 initiative.  +1 spell known/can cast per level

Unfortunately, I don't have a 2nd Edition character sheet anymore -- I was keeping track of her side effects on a second sheet -- and I threw out my 3rd Ed. Sheet when the GM bailed without warning.

I imagine an arguement similar to the three-headed knight in "Monty Python And The Holy Grail" breaks out on occasion between the three:

"I'm going to cast a fireball!"

"You did that last week, and you singed our manicure!"

"I did not! It was her fault!"

"Mine? Who threw the ice crystals before that?"

"It wasn't my idea...I said 'fireball', but noooooo, you wanted something else..."

"Look, can we settle this another time? The bad guys are getting closer..."

"SHUSH!"

The times the heads? Three times the Intellectual Spellpower, anyone knows that. Unless they're using Charisma for Spellpower instead of Intellect or Wisdom. But hey! 3 heads be charming too. EVERYONE knows that.
